How do I change my name from my married name to my maiden name.

  Margaret, on your profile, you have two fields for "Last Name" and "Birth Surname".  Usually, your maiden name would go into the "Birth Surname" field and your married last name into the "Last Name" field.  If you do not want your married name listed, then you can just delete the entry in the "Last Name" field or duplicate your maiden name in both fields.
